The patient meets all of the following (1, 2 and 3):

Palliative Performance Scale equal to or less than 40% (mainly in bed, requires assistance with ADLs)

Body Mass Index below 22kg/m2
Body Mass Index = 703 X (patient's weight in pounds) / (Square of height in inches)

.
The patient declines or is not responding to enteral or parenteral nutritional support
In the absence of one or more of the above findings, rapid decline or comorbidities may also support eligibility for hospice care.
